Thank you for your interest in The Hathaway School. In addition to the following forms, we will also need a copy of your child's latest physical (must be within 60 days), immunization records, a copy of their insurance card and a birth certificate.

Payment Policy:
To greatly assist in keeping costs down, tuition payment must be received by the last Wednesday of every month to avoid a $25.00 late fee.  There is no monthly invoice.  Late fees must be included with late tuition payments within seven days.  Tuition in arrears past seven days will cause a cancellation of educational services.

Parents who have child care vouchers are expected to pay their co-pays on the second and last Wednesday of each month for the weeks in advance. Your co-pays remain constant so it is not necessary to invoice. If there is a change in your co-pay rate, you will be notified and payment is expected on time. The $15.00 late fee policy applies for outstanding co-payments. Refunds are not given for scheduled time that is not utilized.
